Instructions for Application to Work with Children or Youth

This process must be completed by every volunteer with children or youth each year. 

The Safe Sanctuaries Online Training involves four steps. In order to become a volunteer in the children's or youth programs, please complete all steps.

Complete the Volunteer Application for Working with Children or Youth by printing it from your computer. Mail the original completed form to:

Review the following Safe Sanctuaries forms and indicate your understanding of and intent to use them by downloading and reading through the forms:

Report of Suspected Incident of Child Abuse: Every staff member or volunteer of Granbury FUMC is required by law to report suspected incidents of child abuse in the life of a child or youth. This form is used to document the suspected abuse and for immediate reporting.

Accident Report Form: This form is used to report a minor or major injury to a child or youth under the supervision of Granbury FUMC staff or volunteers.
